INSTRUCTIONS FOR ONLINE - DISTANCE LEARNING COURSE 

Videos and Forms are below these Directions 

1. Watch the Videos

2. Complete the Review & Evaluation Form

3. Sign & Email Review & Evaluation Form to ACASCEU1@gmail.com

NOTE: You must scroll down for VIDEOS, and then COMPLETE THE REVIEW & EVALUATION FORM

• On your Browser type in webpage www.acasceu.com 
• Once on the Main page you will notice a tab on the TOP, look for the Tab labeled  DISTANCE LEARNING ON-LINE” 
• Click on “DISTANCE LEARNING” Tab 
• This will take you to the Video Screen 
• Start Watching Videos 

• Once you have completed watching videos you will Scroll Down, download/Print and complete the 

            Course Evaluation Form that corresponds to what you signed up for (12Hrs/12A, 6A Tech/Ethics, 6B X-ray/Philosophy) You must sign and certified you completed the course work.

• Once you have filled in and completed the Course Evaluation Form Please Scan and then  E-Mail the COMPLETED FORM to acasceu1@gmail.com  
• Colleen will assist you and get your certificate emailed asap. 
• PLEASE Allow UP TO 14 Business Days to receive a Certificate of Completion by E-Mail.

• NOTE:  
•                                     ** YOU ARE RESPONSIBLE TO RETAIN ALL CEU CERTIFICATES FOR 4 YEARS. **
• LOST CERTIFICATE REQUEST MUST BE MADE IN WRITING VIA EMAIL WITH THE EXACT DATE YOU COMPLETED YOUR CEU COURSE.
• A BOARD APPROVED DUPLICATE CERTIFICATE FEE is  $70.00    

* Please only call if after 2 weeks, and you have not received it. Typically, Colleen will get this done in a few days, but please allow her time to reply.
